CONVERSION AGREEMENT

I hereby acknowledge my intent to convert my non-credit professional development course to graduate credits through Wilkes University and Learning Sciences International for the courses outlined below.

TERMS & CONDITIONS

Payment: If Learning Sciences International (LSI) has not received payment in the amount due by the payment due date as outlined above, graduate credit will not be granted for the course intended for conversion.

Coursework: All coursework required must be received by Learning Sciences International no later than the coursework due date as outlined above. Research papers received after the due date will not be accepted. All submitted coursework will be reviewed by a qualified graduate facilitator from LSI. Final grades will be returned to the student and submitted to Wilkes University as the final grade received in the course.

Refund Policy: Monies paid for conversion of a non-credit professional development course to graduate credit will not be refunded under any circumstances.

Grades: Grades for all converted courses will be processed by Wilkes University according to the grading scale listed below.

4.0	A	94-100%	Academic achievement of superior quality
3.5	B+	87-93%	Academic achievement of good quality
3.0	B	80-86%	Academic achievement of acceptable quality in meeting graduation requirements
2.5	C+	75-79%	Academic achievement of adequate quality but below the average required for graduation
2.0	C	70-74%	Academic achievement below the average required for graduation
0.0	F	Below 70%	Failure. No graduate course credit

Admissions: The application for the MSEd: 21st Century Teaching and Learning Program must be submitted by the time coursework is due (August 31, 2008). If the completed application is not submitted by the courswork due date, graduate credit will not be granted for the course intended for conversion.

Change of Address: Learning Sciences International must be notified immediately of any address change.

PROMISSORY NOTE

For and in consideration of the above application, I understand that by checking this box, I am aware that when approved and granted, this will constitute a contract for charges as indicated and that this contract is legal and binding upon me. I have read, understand and agree to all the provisions of this contract as set forth.
